This is the Suhr Riot, a sought-after, boutique high-gain distortion pedal. Generally considered to be the best and most amp-like high gain box out there, at least if you believe forums like The Gear Page.
It has gain, tone and volume knobs, and a 3-way voice switch for changing the tone of the pedal (from slightly mellower, to mid-range bite, to scooped JCM800, in my opinion).
I've found it to be a surprisingly versatile pedal. I tested it against my Fulltone OCD for more medium gain stuff, and preferred the Riot. So there you go!
Has velcro on the bottom, so you can put it straight on your board.
