Te Toka Tumai Auckland is seeking an experienced and committed leader to step into the role of Service Lead / Charge Nurse Manager Needs Assessment and Service Coordination (NASC). This is a key leadership position within Adult Community Services, supporting our vision to deliver high-quality, safe and equitable care to our diverse communities, with a strong focus on Maori health gain and whanau-centred care.

About the role

As the Service Lead / Charge Nurse Manager for NASC, you will provide strategic, clinical and operational leadership to a large, multidisciplinary service coordination team working across hospital and community settings. You will oversee workforce, service delivery, financial performance, clinical safety, and quality improvement.

This is an exciting opportunity for a leader who is passionate about supporting people to remain independent in their own home, who thrives in a complex and dynamic environment, and who is motivated to enable staff to perform at the top of their scope.

You will:

Lead, mentor and develop a team of 35 FTE delivering needs assessment and service coordination across Te Toka Tumai. Strengthen partnerships with primary care, home-care providers, NGOs, and community partners. Ensure safe, effective, patient and whanau-centred care through robust triage, clinical governance and quality systems. Guide service planning, budgeting and operational performance to meet our health targets and community needs. Promote models of care that support self-management, independence and seamless transitions between hospital and community. Champion Te Tiriti o Waitangi obligations and equitable health outcomes for Maori and Pacific peoples.

This position is based across our three community sites: Greenlane Clinical Centre, Pt Chevalier (Kahui Marino) and Auckland City Hospital.

About You

You are an experienced, confident and collaborative leader who can inspire teams, drive improvement and build strong relationships across the health system. You bring compassion, clarity and resilience to your work and are committed to upholding Te Tiriti o Waitangi in meaningful ways.

Essential

Minimum 5 years relevant experience, including at least 2 years in people leadership Relevant Allied health or Nursing Qualification with NZ registration and current Annual Practicing Certificate Experience in a healthcare environment NASC or community health experience Commitment to bicultural practice and equitable outcomes for Maori Ability to lead in a complex, rapidly changing clinical environment Strong communication, coaching and mentoring skills

Desired

Budget and operational management experience Post-graduate or Masters-level study (completed or in progress)
