  • Hamilton with some possible travel across the district
  • Permanent 80 hours per fortnight

About the Role

  • Will have line management responsibility budgetary responsibilities where the position holder will be regularly challenged by a wide range of health, social and budgetary determinates that impact strategic service development and delivery.
  • Develop processes and dashboards with partner agencies to meet/ monitor service level KPIs and outcome measure, providing monthly reports.
  • Lead regional projects in collaboration with district and regional partners.

Key responsibilities of the role includes:

  • Ensure effective communication with Mental Health Service Manager, Health New Zealand | Te Whatu Ora Commissioning Managers and regional partners.
  • Develop and strengthen strategic collaboration for sustainable systems, processes, and pathways within Waikato and across Te Manawa Taki.
  • There will be demand to meet deadlines, deliver measurable benefits and maintain accuracy and quality of information to report service outcomes.

About the Team

The Kotahitanga Service Coordination team is intended to enable people with severe enduring mental illness and addictions (who have previously lived in impatient services and/ or are struggling to maintain wellness in other mental health supported living situations) to achieve any or all of the following:

  • Imagine and plan for a better future including a housing and support pathway.
  • Achieve their goals in terms of work and whanau connections.
  • Minimise the adverse impact of their mental illness on their lives and reduce inpatient reliance.
  • Enhance their ability to complete activities that increase satisfaction and wellbeing.

Key outcomes for tangata whaiora

  • An opportunity to plan and pursue a living arrangement and a life of their choosing.
  • The chance to experience mental health services in Waikato as innovative, progressive and recovery oriented.
  • Fulfilling valued social roles in the community and being active, contributing members of their communities.
  • Will not reside within inpatient, residential or respite settings as an alternative to a more suitable home.

About you

  • Competency with te ao Maori, tikanga, and te reo Maori or a commitment to starting your journey and taking ownership of your learning and growth.
  • Whanau centred, collaborative and partnership approach.
  • Experience working in a mental health, community health or primary care setting.
  • Experience working with Maori and Pasifika groups.
  • Highly developed interpersonal communication and negotiation skills including demonstrated ability to build productive working relationships with colleagues to achieve required outcomes.
  • Ability to take a proactive role with other organisations to achieve sustainable outcomes for tangata whaiora.
  • Registration with the applicable registration board with a current annual practicing certificate.
