ABOUT THE ROLE:

This role is open to those looking for part-time or full-time options.

As a Supported Living Facilitator at Renaissance Group, you will provide person-centred support to individuals in their homes, helping them achieve their personal goals and live independently by:

  • Supporting goal achievement focused on developing life skills that encourage independence, e.g. cooking, budgeting, using public transport, finding a job, paying bills and managing household chores.
  • Facilitate community participation and engagement
  • Enhancing choice and control
  • Providing support that is flexible and respects and encourages the ownership of the disabled person
  • Maintain accurate records and documentation that are in line with company policies.
  • Work collaboratively with families, healthcare professionals and other social networks.
