About the Role

We are seeking a motivated NZ Registered Social Worker to join the Engage Community Allied Health Service, based out of our Hastings office. In this role, you will provide safe, effective and culturally responsive social work assessment and intervention for patients, clients and tangata whaiora with a wide range of conditions, including those with increasingly complex needs. You will be working with clients with psychosocial issues arising out of long-term health conditions and disability and their whanau, out in the community (often in the clients own homes).

You will collaborate closely with interprofessional and multidisciplinary teams, complete risk assessments and provide coaching, education and guidance to support ongoing recovery and independence. Supporting whanau to navigate the PPPR Act (1988) is a core aspect of this role, therefore experience in this area would be an advantage.

The role includes opportunities to contribute to service improvement, professional development, clinical teaching and quality initiatives across the Allied Health Therapies service.

While our service covers Napier and Hastings, the role will primarily be based in our Hastings office.

About the Team/Service/ Location

Youll be part of the engAGE Community Allied Health Service, working alongside a dedicated team of community social workers, physiotherapists, occupational therapists, dietitians, speechlanguage therapists, allied health assistants, and professional leaders.

Based from our community office in Hastings, the team supports people living in the community, including older adults, people recovering from illness, and those with longterm conditions. Youll also work alongside with Geriatricians, CNS Stroke, Maori Health Services, and a wide network of community agencies, PHOs, and general practice teams.

The team is collaborative, supportive and committed to providing highquality, equitable and whanaucentred care guided by Te Mauri o Rongo and Pae Ora principles.

About you

You are a capable, reflective clinician with strong communication and clinical reasoning skills, and a genuine passion for improving health outcomes in the community.

You will have:

  • Social Work Registration Board (SWRB) registration, and a current Annual Practising Certificate
  • A relevant graduate and post-graduate qualification.
  • Clean current full NZ drivers license.
  • Proficiency in Microsoft Office, Word, i.e. (Outlook, Excel, PowerPoint, Internet resources and e-mail).
  • Understand the importance of being a team player.
  • Ability to build rapport and constructive and effective relationships.
  • Positive attitude with problem solving focus.
  • Self-motivated in developing clinical and professional practice.
  • Ability to lead, motivate and support others
  • A commitment to Te Tiriti o Waitangi, equity and culturally safe practice
  • Focus on delivering high quality intervention for the client/patient and whanau.

Desired:

  • Member of ANZASW
  • Knowledge of the PPPR Act (1988) would be an advantage
  • Knowledge of local community support networks would be an advantage
  • Community and/or Health-based Social Work experience
  • Understanding of local tikanga and ability to engage respectfully with Maori clients and whanau
