Location: Community Health Service, Hutt Hospital
Contract: Permanent part time 0.5fte (40 hours per fortnight)

About the role
This is a Health Care Assistant role that focuses on supporting the Transitional Home Support Services by supporting people who have been recently discharged from hospital and need some support to attend to household duties and activities of daily living.
This position is 0.5fte working Monday to Sunday, predominantly 0800-1630, with occasional PM shifts 1230-2100 as required.
You will be providing home help and assisting with personal hygiene, medication supervision and other delegated tasks from the nursing team for people who have recently been in Hospital.
About the Team
Community Health Service team is made up of District Nurses, Specialist Nurses, Admin and Health Care Assistants and provides a variety of services with a focus on quality health care working alongside patients and their whanau to achieve optimal health outcomes.
About you
We are keen to hear from Health Care Assistants with a proven track record of working in the community with a restorative focus who are prepared to work the hours available. HCA qualifications at level 3 are an advantage although we will support the right candidate to complete this.
You will required a Restricted or Full NZ Driver's License for this role.
