We have an exciting opportunity for a Registered Nurse with Mental Health experience to join our team of case managers within our Whakatane Mental Health Services for Older People Community team.

This role/team is dedicated to providing high-quality care to the growing population in the Bay of Plenty. You will collaborate with referrers, healthcare partners, referred clients, their families/whanau, and the wider community to deliver efficient triage and assessments. Your goal is to facilitate and implement tailored treatment plans that promote health and independence within the community.
Key responsibilities include:
Offering advice and support Managing and resolving mental health crises Assisting with transitions along the care pathway for service users and their families Connecting consumers to primary care providers and relevant community services

He Korero mou - About You
The successful applicant will be required to have an excellent understanding of mental health conditions, the Mental Health Act, PPPR Act, understanding of capacity assessment work up, a range of therapeutic modalities, dementia/delirium nursing care.
The successful application will have a minimum of 3 years post graduate mental health experience, preferably with experience in older person's mental health.
